Community Paramedic (Internal Candidates Only)

This role provides emergent, nonemergent, and preventative healthcare to high-risk individuals in the community, focusing on those who are unsheltered or dependent on opioids and other illicit substances. The position involves advanced patient assessment, chronic disease management, medication adherence checks, and connecting patients to necessary social services. The Community Paramedic works closely with first responders, healthcare professionals, and community resources to ensure coordinated patient care and prevent hospital readmissions.
