About this role

The Community Paramedic provides high-quality, compassionate in-home and community-based care to patients who may not require emergency transport but need acute or non-acute treatment, ongoing care, or follow-up support. Working in collaboration with various healthcare teams, the role helps reduce unnecessary emergency room visits, improve patient outcomes, and support chronic disease management and preventive care. This position performs all functions of an Empress EMS NYS EMT-Paramedic.

Responsibilities

Conduct advanced in-home patient assessments and provide urgent medical responses
Deliver treatment in place and support chronic disease management protocols
Collaborate with healthcare teams to implement patient care plans and referrals
