Community Resource Advocate

The Community Resource Advocate serves AMH patients by connecting them to community resources and public benefits to support their self-sufficiency and self-efficacy. The role involves helping patients navigate care within AMH, including referrals to behavioral health and dental services, while maintaining relationships with community partners. This position requires managing a patient caseload, documenting encounters, and utilizing peer support interventions when applicable.
