Community Resource Liaison

The Community Resource Liaison identifies and engages individuals and families who may benefit from the Social Care Network Program, with a focus on addressing food insecurity. The role involves sharing available resources and connecting interested individuals with the care coordination team for eligibility screening and enrollment. This position aims to strengthen access to community resources while building trusted relationships within the local community.
