About this role
The Community Skills Trainer for Cascadia Health’s Intensive Case Management program provides community-based support to adults with serious mental illness, working as part of an interdisciplinary team based at the Plaza Health Center in SE Portland. The role supports long-term, person-centered recovery within a trauma-informed, integrated care model. The position is scheduled Monday through Friday, 8:00 a.m. to 4:30 p.m.
