The Community Support Facilitator supports adults with developmental disabilities and mental health needs to engage in meaningful activities and services in community settings. The role focuses on helping individuals participate as valued community members in alignment with their Person Centered and Individualized Personal Plans. It is a field-based position with substantial time spent accompanying clients in their local communities. The facilitator also ensures accurate documentation and coordination with center-based programs and staff.
