About this role

This Community Support Worker role provides hands-on, complex personal care and safety support to older adults with diverse health needs, including those with mental health instability or mobility limitations. The position involves administering medications, monitoring clinical changes, maintaining structured routines, and coordinating with healthcare providers to ensure the client's stability and dignity within the community.

Responsibilities

Provide daily personal care and safe mobility support for clients with complex needs
Administer medications accurately and monitor PRN implications for health changes
Observe and document subtle behavioral or physical signs of clinical destabilization
Implement de-escalation strategies during periods of acute psychosis or agitation
Coordinate appointments and communicate with external health providers and supervisors
