Complex Case Manager

The Complex Case Manager maintains a caseload of members with complex medical and care coordination needs, reporting to the Case Management Supervisor with clinical guidance from the Senior Director of Case Management. This remote role is responsible for conducting comprehensive assessments, developing individualized care plans, and coordinating care across the healthcare continuum to promote quality outcomes and cost-effective care. The position also ensures appropriate systems are in place to collect and report program activities and outcomes for case management programs.
