About this role
The Lead Care Manager is a full-time role supporting Pair Team’s whole-person, interdisciplinary care model to engage high-need Medicaid recipients (including those with serious mental illness, substance use disorder, homelessness, or complex medical needs). The role focuses on building trusting relationships, coordinating care with clinical teams and community partners, and connecting members to social supports and services. This position is primarily remote with 1-2 community on-site visits per month.
