About this role
The Counselor III (Integrated Team Based Care - Older Adult) is an onsite clinical role based at Woodland Park Health Center in NE Portland serving older adults and individuals with physical disabilities. The position operates within Cascadia’s integrated whole-health model and works as part of a multidisciplinary team to support clients’ well-being. The role follows a Monday–Friday business schedule with occasional community visits and on-call crisis coverage.
