Crisis Outreach Care Manager

The Crisis Outreach Care Manager facilitates enrollment, discharge, and follow-ups while providing crisis intervention and clinical services to Seriously Mentally Ill and Mental Health participants. This role involves responding to crisis calls within specified timeframes, conducting community outreach, and educating stakeholders about behavioral health services and entitlements. The position requires significant time spent outside the office meeting with community members and families to support their return to an effective lifestyle.
