CSR/Care Coordination

This role is responsible for coordinating and completing referrals ordered by providers, including acquiring third-party information and arranging appointments. The position involves tracking referral results, maintaining communication with providers and patients, and ensuring proper documentation in the Electronic Health Record. Additionally, the role supports continuity of patient care and contacts patients overdue for follow-up.
