Bay Aging is seeking a full-time Dementia Care Navigator to serve as the primary point of contact for beneficiaries living with dementia and their caregivers. The role involves managing an assigned caseload, coordinating medical and community-based services, and providing caregiver support under the CMS GUIDE Model. This position reports to the Community Living Programs Director and focuses on developing person-centered care plans and connecting individuals with appropriate resources.
