Director, Extended Care Operations

The Director, Extended Care Operations leads the operational strategy implementation, design, standardization, and performance of CareMore’s non-clinic-based Extended Care programs across markets. This role establishes operational policies, resolves complex issues, and directly leads the Extended Care Coordinator team to improve care delivery for high-risk and frail patients. The position is accountable for operational performance, resource deployment, service reliability, and financial stewardship.
