Discharge Planner and SB Coordinator

This role involves assisting medical and nursing staff in arranging post-hospitalization care and promoting continuity of care for patients transitioning from the hospital. The position requires conducting social assessments, coordinating with community resources, and ensuring seamless patient transitions through effective communication and documentation. Additionally, the role includes developing and implementing activity plans to address patient psychosocial and spiritual needs within the Swing Bed program.
