The position is responsible for initiating discharge plans, completing necessary intake documentation, and conducting proactive discharge assessments to address bio-psychosocial and medical needs. The coordinator also facilitates communication with hospital staff, prepares court-ordered treatment plans, and ensures appropriate post-discharge community referrals and resources are provided.
