Discharge Planner

The Discharge Planner coordinates and facilitates safe, timely, and effective discharge planning for patients, working collaboratively with medical, behavioral health, nursing, social services, and community providers. This role ensures continuity of care by coordinating referrals, appointments, medications, transportation, and community resources to support successful transitions from the facility to the next level of care. The position requires strong interpersonal, organizational, and communication skills to address ongoing healthcare, behavioral health, housing, and community support needs.
