Disease Intervention Specialist

The Disease Intervention Specialist works as part of an interdisciplinary team to coordinate client care and facilitate linkage to services for individuals at high risk for or living with HIV, Hepatitis C, and related sexually transmitted infections. This role involves providing case management, harm reduction education, and care coordination to ensure clients achieve their prevention and treatment goals. The specialist collaborates with community partners, state agencies, and internal teams to maintain compliance and deliver person-centered care.
