About this role
The D-SNP Care Coach supports dual-eligible members by evaluating needs and guiding members and families to appropriate health and social resources to achieve optimal wellness. The role focuses on improving health outcomes and service efficiency for the D-SNP population through ongoing assessment and coordination across providers and external partners. This is a field-facing position with some remote work and travel required.
