ECM Case Manager

The ECM Case Manager provides whole-person, member-centered care management services to Medi-Cal members with complex health and social needs, focusing on individuals impacted by substance use disorders. The role involves outreach, engagement, assessment, care planning, and coordination to help members access medical, behavioral health, housing, and community-based services.
