Engagement Care Coordinator

The Engagement Care Coordinator assists youth hospitalized for behavioral health conditions enrolled in the OhioRISE program by supporting families through the hospital discharge process and connecting them to necessary resources. This role involves educating families and community partners about the program, making referrals for follow-up care, and facilitating Child and Family team meetings until a warm handoff to an ongoing care coordinator is completed. The position requires attending in-person and virtual meetings to complete engagement milestones such as crisis safety plans, assessments, and care plans.
