Enrollment Specialist

The Enrollment Specialist serves as the primary liaison between the PACE Interdisciplinary Team and potential participants, guiding them through the enrollment process and providing education about program services. This role involves conducting intake screenings, determining eligibility, and documenting medical and social needs to establish initial care requirements. The specialist also builds relationships with referral sources, community organizations, and families to support enrollment goals and facilitate participant onboarding.
