Epic Population Health Application Analyst

The Epic Population Health Application Analyst serves as the primary application specialist responsible for the administration, support, optimization, and ongoing maintenance of Epic Healthy Planet and related Population Health applications. This role provides technical expertise in care management, quality improvement, value-based care initiatives, patient registries, and population health workflows to support organizational strategies for improving clinical outcomes and meeting regulatory requirements. The analyst collaborates with clinical and operational teams to design, configure, and optimize Epic functionality, ensuring systems align with best practices and organizational goals for preventive care and chronic disease management.
