Executive, Transitional Care and Community Partnership (Tengah General and Community Hospital)

The role supports the Transitional Care and Community Partnership team in developing, coordinating, and implementing community engagement, partnership development, and population health initiatives in Tengah, Choa Chu Kang, and surrounding communities. The incumbent contributes to building community partnerships, supports place-based population health initiatives, and coordinates programmes to strengthen community capacity and resident participation ahead of the hospital's opening. This position involves close collaboration with residents, grassroots organisations, and partner agencies to foster a nurturing healthcare environment.
