About this role
This role is a registered nurse position focused on facilitating multidisciplinary communication and coordinating patient care throughout hospitalization. The clinician reviews hospital stays for appropriate utilization, recommends levels of care, and acts as a liaison among patients, caregivers, physicians, post-acute providers, and payers to support individualized discharge planning and transitions of care.
