The Facilitator coordinates Child and Family Teams and develops individualized service plans under the supervision of a Program Supervisor. This community-based role involves frequent travel throughout Alameda County to engage with clients, manage case services, and provide crisis stabilization as needed. The position requires effective relationship building, documentation, and collaboration with community agencies to support children and families.
