FACT Therapist

The FACT Therapist serves as a primary provider of community-based mental health treatment and rehabilitation for the severely and persistently mentally ill adult population. This role involves conducting comprehensive assessments, delivering individualized therapeutic services, and developing client service plans to support stable community functioning. The position requires collaboration within a multidisciplinary team to provide intensive, community-based support and cross-training to enhance staff skills.
