Family Case Planner

The Family Case Planner provides casework counseling and support to families with children at risk of placement outside the home within a child welfare preventive services program. The role utilizes the Solution Based Casework model to partner with families, assess their needs, and develop service plans to address identified concerns. This position involves direct client services, crisis intervention, and participation in team conferences to ensure effective service provision.
