About this role
Field Advance Practice Palliative Care Provider (NP or PA) delivers palliative care services primarily in patients’ homes and community living settings while also providing virtual support. The role collaborates with physicians, interdisciplinary teams, patients and families to develop and implement complex, goal-directed plans of care within a designated territory. It requires travel within the service area and use of the Epic electronic medical record for documentation.
