About this role
The Field Advanced Practice Provider delivers in-home palliative care across a designated territory, collaborating with company physicians, patients’ other providers, and family members. The role develops and implements complex care plans based on patients’ goals and clinical status and provides both clinical and non-clinical support. The position is field-based with travel to client sites and occasional office attendance for meetings or training.
