Field Care Manager, Behavioral Health

The Field Care Manager, Behavioral Health 2 at Humana provides care management for members with behavioral health needs by assessing clinical and psychosocial status and connecting members and families to appropriate resources. The role supports transitions of care (including post-discharge and frequent readmissions) and collaborates with internal teams, providers, and community services to promote continuity and cost-effective outcomes. This is a remote position with occasional travel and a focus on serving Kentucky-based members.
