About this role
The Field Care Manager Registered Nurse (RN) provides in-home assessments and care coordination for Medicaid members to achieve and maintain optimal wellness. The RN guides members and families to appropriate community resources, develops and implements care plans, and monitors patient progress while collaborating with internal and external stakeholders. This is a hybrid, field-facing role that requires extensive travel to member homes and care facilities.
