The Float Nurse Case Manager assesses, plans, implements, monitors, and evaluates care options to meet patients' health needs, coordinating providers and resources across the continuum to promote quality, cost-effective outcomes. This role supports increased volume within the Case Management department and ensures timely, appropriate use of services for both patients and reimbursement sources. The position is part-time with rotating day shifts and weekend coverage.
