Forensic Care Coordinator

The Forensic Care Coordinator provides person‑centered case management and care coordination for individuals involved in Wisconsin's forensic mental health system, supporting their treatment, community integration, and recovery. The role involves meeting clients in various settings, developing service plans, coordinating services, and collaborating with courts, treatment providers, and community partners, with a hybrid work environment and travel across Dane, Jefferson, and Walworth Counties.
