Full Time Hospice RN Case Manager

The RN Case Manager provides compassionate hospice and palliative nursing care, coordinating clinical services and supporting patients and families throughout end-of-life care. This field-based role operates across the South Houston area (Pasadena, Texas City, League City, Pearland) and represents the organization in patient and family interactions. The position requires clinical judgment, empathy, and adherence to organizational policies and compliance checks.
