About this role

The Medical Case Manager supports individuals living with HIV and other complex patient populations through comprehensive care coordination, linkage to services, and patient engagement. The role involves collaborating with multidisciplinary clinical teams to assess needs, coordinate referrals for behavioral health and housing, and ensure timely documentation in compliance with Ryan White and HRSA requirements. This position requires field-based outreach, home visits, and maintaining professional boundaries while addressing psychosocial barriers to care.

Responsibilities

Coordinate medical case management services for patients enrolled in the Ryan White Early Intervention Program
Conduct patient outreach, engagement, intake, reassessment, and follow-up activities
Assist patients with linkage and retention in HIV medical care and supportive services
Maintain timely, accurate, and compliant documentation within Epic and CAREWare systems
Participate in field-based outreach, home/community visits, and patient engagement activities

Skills

Qualifications

Bachelor's degree in social work, psychology, public health, health administration, or related field3 years of experience in medical case management or care coordinationExperience with Ryan White services or HIV care preferredCAREWare experience preferredEpic experience preferred
