About this role
The Medical Social Worker provides social work services to patients and families in a hospice care context, supporting psychosocial needs and care planning. The role works closely with an interdisciplinary team and community resources to support patients’ wellbeing and continuity of care. This is a part-time position (15 hours/week) with occasional coverage up to full-time hours when needed.
