Health Coordinator- PT/Swing

The Health Coordinator supports the mission of Pioneer Human Services by coordinating and monitoring client physical and behavioral health services within residential treatment facilities. This role ensures continuity of care by facilitating communication between clients, nursing staff, medical and behavioral health providers, and program staff. The position involves supporting the intake process, coordinating appointments, assisting with medication monitoring, and providing direct client support in a structured residential environment.
