About this role
The Health Home Care Manager provides supportive Health Home care management, coordinating services for individuals identified as high users of services to improve access to medical, mental health, social, educational, and financial resources. The role develops and maintains written care plans and manages enrollment and oversight for assigned cases, including 24/7 oversight as needed. This position is based at Unity Hospital of Rochester within the Care Management department.
