Health Information Representative

The Health Information Representative ensures CMS program compliance by creating and reviewing Plans of Care and maintaining Home & Community Care patient records. This role collaborates with clinicians and providers to resolve medical record issues impacting the revenue cycle and ensures documents are properly scanned, indexed, and retained. The position supports the integrity of the medical record while working within a patient-focused care culture.
