Health & Wellbeing Facilitator

The Health & Wellbeing Facilitator supports individuals with physical or mental health challenges to overcome barriers and achieve sustainable employment through personalized interventions and group workshops. This role involves conducting assessments, developing tailored action plans, and collaborating with employment teams and external stakeholders to promote long-term wellbeing and independence. The position is based in Southampton, Portsmouth, and Basingstoke with a hybrid working arrangement.
