Hennepin County Case Manager

The Hennepin County Case Manager at Fraser provides client-centered case management services to individuals receiving CADI/BI and DD waiver services, advocating for clients and connecting them to community resources. The role involves managing a caseload of approximately 35 to 43 clients, meeting with them two to three times per year, and ensuring culturally responsive practice. This position supports individuals with developmental disabilities and serious mental illness, requiring strong organizational and documentation skills. The role is based in the Twin Cities Metro Area with travel to meet clients 2-3 times per week.
