HERO Case Manager

The HERO Case Manager builds trusting relationships with individuals experiencing homelessness or housing instability, including those with a history of opioid use, to help them secure affordable housing and maintain stability. This full-time role involves community-based outreach, advocacy, and collaboration with internal teams and community partners to address barriers to housing and support client success. The position requires travel throughout the Greater New Haven area and a valid driver's license.
