HIV/PrEP Navigator

The HIV/PrEP Navigator coordinates patient navigation, care coordination, and community outreach to connect individuals living with or at risk for HIV with prevention, treatment, and supportive services. This role manages intake, referrals, scheduling, and ongoing follow-up while addressing social barriers to care and assisting patients with insurance, medication access, and financial assistance. The Navigator works collaboratively with clinical teams and community partners to provide patient education, perform Medical Assistant duties, and coordinate outreach events and partnerships.
