About this role
The Hospice Home Care Nurse provides nursing and supportive care for terminally ill patients and their families, identifying patient, family, and facility care needs. The role reports to the Team Manager and supports continuity of care across home, facility, and inpatient settings. The nurse collaborates with interdisciplinary teams and the primary physician to implement and monitor individualized care plans.
