The Hospice Home Care Nurse provides compassionate end-of-life nursing and supportive care for terminally ill patients and their families, primarily in home or facility settings. The role reports to the Team Manager and collaborates with an interdisciplinary group to ensure holistic, patient-centered care across care settings.
