About this role
The Home Health & Hospice Care Navigator at Otterbein drives post-acute skilled Home Health service census and referral growth across assigned Ohio territories (Bellefontaine, Marysville, Dublin, New Albany). The role partners with Otterbein communities, skilled nursing facilities, and hospitals and conducts community outreach to promote services and ensure smooth patient transitions. This position is mission-focused within a faith-based, nonprofit senior care organization and is full-time (40 hours/week).
