About this role
The Home Health & Hospice Care Navigator promotes growth of Otterbein’s Home Health and Hospice ministries across assigned Ohio territories (Bellefontaine, Marysville, Dublin, New Albany). This full-time role represents Otterbein to hospital systems, skilled nursing facilities, community organizations and referral sources to increase post-acute skilled home health census and support the nonprofit’s mission-driven care. The position is community-facing and connected to Otterbein’s broader clinical and ministry teams.
