About this role

The Home Health & Hospice Care Navigator is a field-based role responsible for driving post-acute skilled Home Health census growth across Bellefontaine, Marysville, Dublin, and New Albany, Ohio. The position builds and maintains referral relationships with hospitals, skilled nursing facilities, senior living communities, and community organizations to increase admissions to Otterbein Home Health. This role represents Otterbein SeniorLife’s faith-based, not-for-profit Home Health and Hospice ministries in the community.
