About this role
Licensed Practical Nurse (LPN) for ThedaCare Home Health providing patient-centered care in patients' homes as part of ThedaCare’s community-based services. The role is full-time (40 hours/week) with a regular M–F daytime schedule, some PM on-call shifts, and a holiday on-call requirement. Positioned in the TC at Home program in Appleton, Wisconsin, the role supports ThedaCare’s vision of proactive, value-driven health care.
